On home page there is download of Long Beach Notice of Appeal form and a generic form for download. Its a basic form.

You can make simple forms for court and for the city using a word program. Fancy is not a requirement.

Also, download the progress chart from the home page https://administrativecitation.com/downloads/

If your city does not have a form, or even if they do. You can simply copy the single notice page substituting your information. Send it to the city by priority mail or email. You need to have a record of when you sent it and when it was received.

Do not check boxes that cabin you into your argument at the hearing. Just appeal because you believe you are not responsible or guilty of any violation alleged by the city whether factually, or legally.

First you file a notice of appeal to the city. When you lose you file a notice of appeal to superior court.
